Personal Information Collected:
We collect appropriate and pertinent personal information whenever you engage in one or more of the following with Profile Legal Services Inc. directly through Human Resources Department or management. Personal information may be collected verbally or non-verbally, through the following methods:
1. Application for employment
2. Inquiry regarding employment availability
3. Personal employee form
4. Any communication/conversations where information is offered.
The purposes will be limited to those which are related to our business and which a reasonable person would consider appropriate in the circumstances.
We will make a reasonable effort to identify the purpose of collection to the individual from whom the Personal Information is collected. We will state the purpose in such a manner that an individual can reasonably understand how the information will be used or disclosed
Personal information may include one, some, or all of the following information about you:
a. Name, address, and telephone number
b. Personal facsimile number and email address
c. Occupation (current and past)
d. Work history
e. Pay history
f. Language preference
g. Date of birth (we only require to know if you are over 18 and under 65 for purposes of Canada Pension) or actual date of birth for benefits purposes or if requested by WSIB in the event of a workplace injury
h. Social Insurance Number, work permit number, and other identification
i. Banking information for direct deposit
j. Driver’s License number or photo identification
k. Marital status only if needed for benefits application
l. Personal emergency contact information
m. Hours worked
n. Physical limitations and capabilities for job offered
o. Credentials to support training and/or education
Please Note: Follow Up Social Insurance Number and/or work permit number is required by law at time of hire.
Safeguarding Personal Information:
We use your personal information for the purposes listed below. Access to your personal information is restricted to the Human Resources Department and President, who need access to perform their work.
Use of Personal Information:
Your personal information may be used for one or more of the following reasons related to providing customer service to you:
1. Provide employment confirmation
2. Obtain personal benefits
3. Process payment by cheque or direct deposit
4. Follow-up to obtain your feedback on our level of service
Personal Information will only be collected, used, or disclosed with the knowledge and consent of the individual, except where inappropriate.
The way in which we seek consent, including whether it is express or implied, may vary depending upon the sensitivity of the information and the reasonable expectations of the individual.
